Established January 2011.  In-home non-medical personal care service, assisting seniors with intensive physical and emotional support. Performs activities-of-daily-living (ADLs) including, but not limited to, fall prevention assistance, ambulation assistance, transferring assistance, end-of-life/hospice assistance, memory care/cognitive/behavioral assistance, supplemental medication management, incontinence care, toileting, range-of-motion exercise assistance, repositioning, skin care, feeding assistance, and providing socialization with the senior. Allows the senior to stay at home or assisted living facility, in their comfort zone, rather than live in an nursing home. We believe in tailoring our care to the individual senior, the senior's family, the environment in which the senior lives, and the systems of our referral partners and care collaborators (like hospice agencies, assisted living facilities, rehabilitation hospitals/home health services, and independent senior living residences). We also believe in matching the best aides for each assignment, based on skill set, personality, and connection with the senior, including as much continuity and consistency of aide assignments as possible.
