The MOH Office for Healthcare Transformation (MOHT) is Singapore’s Ministry of Health innovation office, established in 2018 to drive system-level transformation in healthcare. It operates at the intersection of policy, operations, and technology to develop and scale care models that improve health outcomes and deliver greater value for Singaporeans. Based in the Central Region of Singapore at 1 North Buona Vista Link, it is a mid-sized public health organization. MOHT collaborates with MOH, public healthcare clusters, and community stakeholders to co-design, test, and deploy solutions that are ready for real-world use and sustainable mainstream adoption. Its work centers on three priorities: Primary Care Transformation, Mental Health Innovations, and Frailty Management to help keep people healthier for longer. A recent development is the 2026 launch of GRAECE, a guided approach for AI-enabled clinical excellence.
