Granite Health is a collaborative network formed by six of New Hampshire's largest health systems to reshape how care is delivered in their communities. The member hospitals are Catholic Medical Center, Concord Hospital, Exeter Health Resources, LRGHealthcare, Southern New Hampshire Health, and Wentworth-Douglass Hospital, and they work together to provide high-quality, accessible care while pursuing better population health. Based in Concord, New Hampshire, the organization centers on placing patients and families at the heart of care. Operating in the hospitals and health care sector, Granite Health functions as a coalition that seeks to coordinate services and improve health outcomes across the state. In 2017, Granite Health and its five hospitals launched the Zero Left Campaign to promote safe disposal of unused medications, install medication take-back boxes in facilities, and train providers to prescribe opioids in alignment with standards adopted by medical boards.
