TPAN is a Chicago-based nonprofit in the individual and family services sector that concentrates on HIV care and related social supports. The organization has served Chicago for more than 37 years and recently formed a partnership with Vivent Health to deliver integrated HIV care. The collaboration aims to address longstanding disparities in HIV healthcare and contribute to Illinois' Get to Zero goals. The partnership signals a shift toward a coordinated, culturally competent care model for people affected by HIV across the city, supported by a small to mid-sized team. TPAN, together with Vivent Health, plans to open an HIV Health Center in the Edgewater neighborhood of Chicago, which will house medical care alongside services such as a food pantry, dental, legal aid, housing assistance, and pharmacy services; the medical clinic is slated to open in August 2024, with the food pantry planned for early 2025. The new center builds on existing programs, including case management, prevention services, substance misuse treatment, and LGBTQ+ youth support, to broaden access to HIV care for residents citywide.
