North Western Melbourne Primary Health Network (NWMPHN) is a mid-sized health network based in Docklands, Melbourne, Victoria. It works to strengthen primary health care and connect services across the health system. Its core aim is to increase the efficiency and effectiveness of medical services for patients, especially those at risk of poor health outcomes, by improving care coordination so people receive the right care in the right place at the right time. In March 2025 NWMPHN released its Reconciliation Action Plan for 2025–2027, marking a concrete recent development. The organization serves local primary care providers, hospitals, and communities as part of its broader aim to support integrated care delivery in the northwestern Melbourne region.
