Exalta Health is a community-based nonprofit health organization based in Grand Rapids, Michigan. Established in 1996 by local physicians, nurses, social workers, pastors, and businesspeople, it began as Health Intervention Services to provide faith-centered medical care for underprivileged women. The organization later broadened its scope to serve anyone without access to health care and began integrating behavioral health with primary care. Since 1998 it has operated from the Burton Heights area, continuing to serve the West Michigan community as a resource for accessible health care. In 2017 the organization adopted the Exalta Health name to reflect its broader mission and ongoing commitment to addressing social determinants of health and delivering care where people live and work, all grounded in its faith-based values. As a small to mid-sized nonprofit in the health sector, Exalta Health emphasizes measuring its impact on patients and the community while maintaining its core focus on improving community health.
