Community Care Plan (CCP) is an insurance provider based in Fort Lauderdale, Florida, formed in 2000 as the South Florida Community Care Network, the first safety-net hospital-owned Provider Service Network. The organization serves Medicaid and Florida Healthy Kids populations, uninsured programs, and self-insured employee health plans, collectively reaching more than 165,000 enrolled lives. CCP is owned by Memorial Healthcare System and Broward Health, and it maintains accreditation from the National Committee for Quality Assurance for its Medicaid and Florida Healthy Kids plans, as well as full accreditation from the Accreditation Association for Ambulatory Health Care as a health plan. In addition to its core plans, CCP launched a new division in 2025, 22 Health, to offer ACA marketplace plans to Broward County residents starting in 2026. The company operates a virtual Community Resource Center, HEART, providing free events and resources focused on health, education, access, and tools to support the South Florida community.
