In October 2024, Clinica Family Health (Clinica) and Mental Health Partners (MHP) merged into one comprehensive healthcare nonprofit organization: Clinica Family Health & Wellness. Together, we are a Federally Qualified Health Center (FQHC) and Certified Community Behavioral Health Clinic (CCBHC) providing essential medical, dental, mental health, and substance use care to residents of Adams, Boulder, Broomfield, and Gilpin counties.

As separate organizations with common purpose, Clinica and MHP collaborated closely for over 30 years. A hallmark of this partnership was the launching of our innovative whole person healthcare model: the Integrated Health Home (IHH) program in Boulder. Together, we also opened co-located health clinics in Nederland and Black Hawk to better serve our rural mountain communities. 

After experiencing the success of these programs, Clinica and MHP leadership began talking about how we could increase our impact by more effectively breaking down barriers and expanding access to high quality, affordable healthcare. It became abundantly clear that uniting our organizations—leveraging over 100 years of combined experience and expertise in physical and behavioral health—was the strongest way to effect meaningful change and improve health outcomes for the populations we serve.
