Not all hospice providers are the same. Much like the differences you’ve experienced between various medical providers over your lifetime, hospice care will mean different resources among different organizations. Center for Hospice Care provides coordinated, competent, compassionate care that is centered on the needs of patients and their families. The nonprofit Center for Hospice Care, has been serving New London County since 1985. We provide two care models: - Palliative Care — designed for those who wish to continue aggressive treatments while at the same time controlling symptoms to achieve a comfortable quality of life while living with an illness that limits life expectancy. - Hospice Care — for those with advanced, end-stage illnesses, who are seeking to control pain and their environment, whether at home or at a Skilled Nursing Facility. In addition, Center for Hospice Care provides a network of support to patients and their families that includes: - Primary Physician - Medical Directors (medical management and plan overview) - Nurses - Hospice Aides (personal care) - Social Workers - Pharmacists - Volunteers - Complementary Therapists (pet therapy, massage therapy and related specialties) - Chaplains - Bereavement Services (open to the public as well)
