America's PPO was founded in 1982 under the name Family Health Plan. The term Preferred Provider Organization (PPO) was coined for the company by Dr. Paul Ellwood of InterStudy, the same man who coined the term HMO. 

America's PPO is a regional PPO – it's provider network includes Minnesota, North and South Dakota, northern Iowa and western Wisconsin. The company holds direct contracts with all the providers in its network.

The company is part of the Araz Group of companies, headquartered in Bloomington, MN.
